I was not happy with the first patch; due to the way qmail-send works
it'll lead to exponentially increasing delays for messages and generally
inconsistant delivery times so I have removed it.

the recipeitnreload patch was a simple change for someone who asked for it.

the sendmail-b-flags patch was a response to wining by someone that qmail's
sendmail wrapper did not support the -b flags. DUH this was simpler still
to write

Nick Leverton did the holdremote patch -- it's here because I once used it
and it might be useful to others.
